    What Enclomiphene Results Look Like on Paper

    Men who search for enclomiphene results usually want two things: how much testosterone actually rises, and whether they feel different. The first question has good data from clinical trials. The second has almost none from trials, but plenty of reports from men who have posted their blood work online.

    On paper, the pattern is consistent. In the phase IIB trial by Wiehle and colleagues (Fertility and Sterility, 2014), average total testosterone went from 217 to 472 ng/dL on 12.5 mg daily and from 210 to 406 ng/dL on 25 mg over about 3 months, while men on placebo stayed flat at around 200. In the two phase III trials reported by Kim and colleagues (BJU International, 2016), testosterone rose from 203 to 446 ng/dL and from 213 to 413 ng/dL over 16 weeks.

    In both cases men started below 300 ng/dL, the usual cut-off for low testosterone, and ended in the normal range. A 2025 meta-analysis in Archives of Endocrinology and Metabolism, pooling 10 trials of clomiphene and enclomiphene, estimated an average gain of about 274 ng/dL over placebo.

    In the phase III trials, testosterone reached a steady level above 400 ng/dL after 4 weeks of treatment (Kim et al., BJU International, 2016).

    These are averages, and individual results spread widely. In the Wiehle 2014 trial, the standard deviation of the final testosterone value was over 160 ng/dL in both enclomiphene groups. That means some men finished well above 600 ng/dL and others barely moved.

    LH and FSH explain the rise. On 12.5 mg daily, LH went from 4.4 to 8.9 mIU/mL and FSH from 6.4 to 11.5 mIU/mL over 3 months in the Wiehle 2014 trial. Men on testosterone gel saw both fall.     Fertility Results: Sperm Counts Before and After

    For men who want children, sperm results matter more than testosterone. This is where enclomiphene results stand apart from testosterone therapy.

    In the two phase III trials, sperm concentration rose by 11.7% and 15.2% on enclomiphene over 16 weeks, while it fell by 56.6% and 32.8% on AndroGel 1.62%. In the phase IIB trial, 54% of men on testosterone gel became oligospermic, with counts under 15 million per mL, compared with 15 of 103 men (14.6%) at any point on enclomiphene.

    The small Kaminetsky study (Journal of Sexual Medicine, 2013) reported the most striking before and after figures. Enclomiphene raised sperm counts in 7 of 7 men at 3 months and 6 of 6 at 6 months, to concentrations of 75 to 334 million per mL. Testosterone gel failed to raise counts above 20 million per mL in any of 5 men at 3 months.

    What Men Report on Forums

    Outside trials, men share their own enclomiphene results on forums such as ExcelMale, MESO-Rx and AnabolicMinds. What follows are individual reports, not data from studies, and they come from people who chose to post, which may tilt toward strong reactions in both directions.

    The most repeated pattern is fast doubling. Several different users report testosterone roughly doubling within about a month on 12.5 mg daily:

    1. 1One ExcelMale user went from 220 to 330 ng/dL at baseline to 630 ng/dL after one month.
    2. 2Another, aged 59, wrote that he "went from 300 to 780 in a less than month".
    3. 3A MESO-Rx user coming off a steroid cycle reported 400 to 920 ng/dL in about 30 days, with "zero sides".
    4. 4Another ExcelMale user posted 280 to 555 ng/dL, with free testosterone rising from 6.6 to 22.8 pg/mL.

    The timing of these reports fits the trials, where levels settled after about 4 weeks. Most users also report taking their first follow-up labs at around that point. Not everyone responds: one user reported "no change at all" after 27 days, and a man who switched from long-term TRT measured only 182 ng/dL on 25 mg every other day.


    Good Numbers, Mixed Feelings

    The biggest gap between trial results and user reviews is how men feel. The trials measured blood levels, not symptoms, and that was one reason the European Medicines Agency refused approval in 2018: the studies did not show whether libido, bone strength or other symptoms improved.

    User reviews echo that gap. A moderator on ExcelMale summed it up: "Guys frequently end up with good numbers on enclomiphene, but the subjective results are more hit or miss." Other users describe "great numbers, but did not feel the way I should have" and "a feeling of being flat". Several report low libido despite normal testosterone, while others describe stronger erections and clearer thinking.

    One likely factor is estradiol. As testosterone rises, some converts to estradiol, and both too much and too little can affect libido and mood. Some users describe nipple tenderness early on.     How to Read Your Own Before and After Labs

    A fair assessment of enclomiphene results needs the same conditions before and after:

    1. 1**Same time of day.** The Wiehle 2014 trial drew blood at 9:00 am in a fasting state. Afternoon tests can read lower.
    2. 2**Same lab.** Different labs use different reference ranges.
    3. 3**Full panel.** Total and free testosterone, LH, FSH and estradiol, not testosterone alone.
    4. 4**Right timing.** About 4 weeks after starting or changing the dose, when trial levels had settled.
    5. 5**Symptoms written down.** Libido, energy, mood and sleep before and after, since numbers alone did not predict how men felt.
